Lambing Report and Registration Application - MS Excel File

To reduce the paperwork required we have combined the lambing report and the registation form. All lambs should be reported on this form. If you choose to register the lambs put a Y in the registration field, include the federal ID and all the other required data and send it in with registration fees of $10/ewe lamb and $5/ram lamb. If a lamb is born dead put stillborn as the lamb name. If it has died put the date of death and cause of death in the lamb name.

Member Information

You may pay for your annual dues via Paypal. You may also donate money to the Association.

1 Year Dues. $20.00 plus Paypal fee of $0.70	We've had some problems with our on-line dues payments so that is disabled until I can fix the problems. In the meantime, you can send in a donation and put a note that it is dues and I'll make it work that way. Sorry for the problems.
Donation

You may also pay for your lamb registrations or transfers of registered sheep via Paypal. Please send the completed registration form via e-mail to info@blackwelsh.org For transfers please send the registratin certificate with teh back filled out. A scanned and e-mailed copy is acceptable or you can send it by regular mail.

To register more than one lamb edit the quantity to be the number of lambs you wish to register.

Transfer a Sheep $5.00 plus Paypal fee of $0.40
